Once filled out, SAVE and DOWNLOAD the form.
Save the form as [YOUR NAME] - TRIP TO [INSERT DESTINATION + YEAR] (For example: Amy B. - Trip to Bahamas 2022)
Email the completed form to travelbycynsae@gmail.com using the title [YOUR NAME] - CC AUTHORIZATION FORM (For example: Amy B - CC AUTHORIZATION FORM)

The customer also has the choice to make payments via Cashapp, Venmo and PayPal, all title "Cynsae."
